If I fly out of clouds, fog, any moisture 4000 ft altitude in temperature -5 to +5 with full carb. heat, do I have risk of engine failure? Can I expect blade icing?
=== with full carb heat you don't have a risk of engine failure because of carb icing - may be some different reasons not depends on the weather conditions. Blade icing depends on temp. and humidity (incl. dew point)
With full carb heat I have about 23 inches of power in these temperatures or less because of carb heat?
=== your MAP depends on temperature IN CARB, not in air out. So use pressure with carb temp.
What condition I never should to fly?
